Outlines StoryCorps’ code of conduct as related to respect and the right to privacy of participants.

Participant Data Sheet
Provides basic demographic and contact information to StoryCorps, the American Folklife Center at the Library of Congress, and other archive partners.

StoryCorps General Release Form
The General Release gives StoryCorps ownership of the interviews and associated materials, including photographs, which allows StoryCorps to use these materials in any way that is consistent with StoryCorps’ mission. This includes using the interview and photographs in radio broadcast, in books, on our websites, and in StoryCorps promotional materials (online and in print). StoryCorps can also share the interview with partner organizations (such as museums and libraries) and researchers.

Library of Congress Only Release Form
The Library of Congress Only Release allows StoryCorps to send your interview and photograph to the American Folklife Center at the Library of Congress for archiving. The interview will be accessible to the public and researchers at the American Folklife Center. The Library of Congress does not make copies of the interviews and does not share your personal information with anyone. Please note that if you sign the Library of Congress Only Release, StoryCorps cannot use your interview for any purpose. Individuals can share their interviews with whomever, but StoryCorps will not provide community partners with copies of Library of Congress Only interviews.

Facilitator Debrief Form
Captures feedback about each interview and provides valuable information to partner organizations and Legacy staff.

Helps StoryCorps catalog the interview and make it more accessible to future listeners.
